Outreach to Recovery
Outreach to Recovery is an intensive case management program that provides ongoing treatment and support to individuals who are experiencing significant difficulties due to a serious mental illness and possibly also due to substance abuse or dependence.
Why would I need/ want this service?
If you have a mental illness and a substance abuse problem that are impacting your ability to function adequately in your daily life and need support and treatment to address these issues.
Who is this service geared towards?
Individuals 18 and over with a serious mental illness who also have a concurrent disorder; that is, who have a substance use problem.
When can I take this service?
Once your referral is received, you will be contacted for an appointment to discuss your situation and your needs. It will then be determined as to whether the level of service provided by Outreach To Recovery is appropriate to your needs. If so, service will generally start within two weeks of that decision.
What will this service offer me?
You will be assigned a case manager who will work with you to identify your goals and ways to address the difficulties you face. Our psychiatrist will take over your treatment. Services will be offered in both office and community environments. Support from the team is available 365 days a year. An after hours crisis line is available to all clients of Outreach to Recovery.
