Older Adult Rehabilitation
Older Adult Rehabilitation is a 19 bed male and female inpatient specialized mental health program for individuals over the age of 65 with severe, persistent mental health problems which may be associated with acute behavioral changes. These services are delivered in a collaborative, client centered individualized manner to promote best practices principles and psychosocial rehabilitation philosophy.
Why would I need or want this service?
This service is ideally suited for individuals over the age of 65 with a chronic or relapsing mental health problem who may require longer term rehabilitation before transitioning to a safe community setting. Other individuals over the age of 65 may also have acute behavioral changes associate with mental health problems and will require a short term hospitalization for assessment and stabilization in order to return to community living. These individuals will benefit from psychosocial and recovery focused care in a hospital setting. The primary goal is to promote living successful, satisfying lives in the community.
What services are provided?
Older Adult Rehabilitation provides comprehensive, individualized care which includes assessment, treatment, stabilization, and transitional support. Psychiatric rehabilitation interventions focus on the development of client skills and development of environmental supports for clients to return safely to community living.
Who provides these services?
Services are offered by a variety of skilled clinical professionals who work to facilitate clients' recovery. The clinical professionals who may assist you include: psychiatry, medicine, nursing, psychology, occupational therapy, social work, leisure lifeskills instructors, physiotherapy, spiritual care and dietary.
How do I access this service?
To access this service the individual would need to talk to a physician who would contact the Older Adult Rehabilitation physician for a consult. The referring physician would then contact the individual requesting access to inform them of their suitability for the service.
