The Senior's Behavioural Health Outreach Team is committed to providing quality non-pharmacological behavioural health interventions and case management services to seniors that have a mental illness or dementia. Care and follow-up is provided in a caring and compassionate manner respecting client privacy and confidentiality.
About Us
The Seniors' Behavioural Health Outreach Team conduct assessments, interventions and follow-up that focus on the mental health status of the senior which includes behavioral issues, functional decline, safety, caregiver stress, education needs, cognitive issues using a P.I.E.C.E.S framework.
Who is the Senior's Behavioural Health Outreach Team for?
Individuals 65 years and over that have behavioural issues. Referrals related to individuals less than 65 years of age are reviewed on a case by case basis.
Who are the care providers of the Seniors' Behavioural Health Team?
Clients with the Seniors' Behavioural Health Outreach Team are cared for by knowledgeable and experienced regulated health care professionals such as social work, nursing, and occupational therapy. Each client is assigned a case manager who oversees their care and follow up.
How Do I Access the Seniors' Behavioural Health Outreach Team?
Referrals to the program are sent to the Seniors' Mental Health Intake Facilitator.
