Occupational Therapy - In Home Functional Abilities Evaluation

The In Home Functional Abilities Evaluation is completed by a registered Occupational Therapist who visits the home to evaluate what Home Maintenance or Attendant care services may be required. A Form 1 may also be completed, as needed, with appropriate follow-up with assistive devices and education as recommended and approved by the referent.
