Answers

1. What if I don't have a family physician for a referral?

Referrals can be made by attending a walk-in clinic, your local hospital, community health clinics, Community Care Access Centre or other health care services.

2. Is there a charge for seeing the Nurse or Doctor?

This program is covered by OHIP and there is no cost involved.

3. What does the nurse do at my house?

The nurse will complete an overall health screen. The nurse will take a medical history, review your medications, perform a physical examination, take your blood pressure and order any further tests if needed.

4. Why will I see the doctor at the clinic?

The Geriatrician reviews the assessment of the client and focuses on the medical and cognitive needs of the clients.

5. Can I bring a family member to the appointment?

We recommend that a family member attend the home assessment and the clinic appointment.

6. What do I need to bring to the clinic appointment?

It is recommended that your medications be brought to the clinic for review and also your OHIP card.

7. Will the doctor now become my regular doctor?

The Geriatrician is a consultant and you will continue to see your family physician. A report of your clinic visit will be sent to your family physician.

8. Will I see the doctor again?

The Geriatrician will book any follow-up visits if required.

9. What if I live in Long Term Care or assisted living?

The physician at the Long Term Care Home can make a referral for the Nurse Practitioner and Geriatrician.

10. What if I'm not a senior?

Clients are generally 60 years of age and over. Clients under 60 years of age can be seen by referral.
