Outpatient Rehabilitative Care

Rheumatic Diseases Program

The Rheumatic Diseases Programs helps clients living with arthritis to maximize their function and improve their quality of life. Clients can be referred by the Physician and Nurse Practitioner for services. Clients who are eligible have a primary diagnosis of a specific type of arthritis (inflammatory, osteoarthritis) for the Rheumatic Diseases Day/Single Discipline and GLA:D. Fibromyalgia diagnosis is required for Living with Fibromyalgia.

The services offered include:

Rheumatic Diseases Day Program

Provides an interdisciplinary care approach to clients for individualized assessment, treatment and education. The primary objective is to promote self-management of the condition with a better understanding of their arthritis and prevention strategies to the client, better enabling him/her to manage their disease process.

Rheumatic Diseases Single Discipline

Offers assessment and treatment by a single discipline (Occupational Therapy or Physiotherapy) to provide short term rehabilitation to clients living with arthritis.

GLA:D

Provides an eight week education and exercise program for clients living with osteoarthritis that are not surgical candidates or those who prefer conservative management.

Living with Fibromyalgia

This program is offered twice per year in person or in a virtual format. Provides education and support for individuals living with fibromyalgia with a focus on self-management.
