The Personal Health Information Protection Act (PHIPA) is Ontario's health specific privacy legislation. PHIPA governs the manner in which personal health information may be collected, used and disclosed within the health sector. It regulates health information custodians, as well as individuals and organizations that receive personal health information from custodians.
You have the right to access and request a copy of your own personal health information. If you wish to do so, please contact Health Records by calling 807-343-2454. or by email SJCG.Privacy@tbh.net.
