Extension for Community Healthcare Outcomes

Project ECHO links expert interdisciplinary teams with primary care clinicians. Through participation in ECHO, healthcare providers form communities of practice, acquiring new skills and knowledge, and increasing overall confidence. Our goal is to support primary care providers in diagnosis and management of their chronic pain patients.

North West Regional Palliative Care Program

North West Regional Palliative Care Program mobilizes, strengthens the healthcare system to improve access to safe, comprehensive, and high quality palliative care for all residents of Northwestern Ontario.

The North West Regional Palliative Care Program (RPCP) is responsible for supporting access to high quality palliative care across the Northwest. The RPCP strives to create an integrated and coordinated palliative care system for everyone. Regardless of the care setting, individuals and their families will receive care that is person-centred, sensitive to diversity and culture, supportive of an interprofessional team approach and close to home. The RPCP uses a community capacity building approach where existing strengths and resources within community are built upon and stronger relationships are created amongst care providers.

North West Regional Rehabilitative Care Program

The North West Regional Rehabilitation Care Program works with healthcare professionals and system partners to support the provision of an integrated system of care that is person-centred and culturally safe for clients at each stage of their physical rehabilitation journey.

North West Regional Wound Care Program

The North West Regional Wound Care Program is a regional program that serves to standardize and coordinate wound care practices across the health care continuum in the North West region, and to optimize clinical outcomes for our patients.

The program supports the development of coordinated, high quality, evidenced-based care and the improvement of patient experience for all wound types across the spectrum.
