St. Joseph's Care Group - Strategic Planning 2024-2028

St. Joseph's Care Group is developing our 2024-2028 Strategic Plan and we want you to be involved!
We are working with Santis Health, an organization that specializes in strategic planning to develop a comprehensive and informed strategic plan. As part of this work, Santis is conducting a survey to gather feedback from staff and physicians, volunteers, clients/resident/tenants, care partners, community partners, and the general public. Information provided by you in this survey will help us determine what we should focus on achieving over the next four years.

Why We Participate in Strategic Planning
St. Joseph's Care Group engages in strategic planning to review our mission, vision, and values, and determine objectives and define a pathway to achieve our goals. Think of our Strategic Plan as a roadmap, it illustrates specific goals that we'd like to achieve by the end of the four-year period. These goals influence our initiatives, projects, and decisions.
One Survey, Four Streams
The questions in this survey will be geared to you based on your experience with St. Joseph's Care Group. If you fall under more than one category, please feel free to complete the survey multiple times under each applicable category.
Individually completed surveys will remain confidential by Santis Health. Information provided to St. Joseph's Care Group will be compiled into statistics and themes.
At the end of our survey, you have the opportunity to provide your email address to receive future updates on the Strategic Planning process. This information will be provided as a list to St. Joseph's Care Group separate from survey data.
