Referrals can be made to our programs by physicians, Parole/Probation officers, CAS workers, and Addiction and mental health professionals within Thunder Bay and the surrounding area. SMSC does accept referrals from out of province, provided they have a valid Health Card from their province of residence.
Clients can also self- refer by calling and speaking with an intake worker, however they will be directed to contact a professional in their area, if they are from out of town, for the assessment paperwork.
In order to refer your client to our service, a referral package must be completed.
Counsellors trained to complete the approved assessment (GAIN), will be asked to submit them to us
for review, along with the referral package.
Once assessment paperwork has been received, a treatment plan with be discussed with the client, which may involve individual counseling, group counseling, and/or residential counseling.
SMSC Residential program DOES ALLOW clients who are stable on Methadone, to attend treatment.
